Understanding the Anatomy of Tonsils

The tonsils are involved in the lymphatic system that serves as a network of organs and tissues within the body. These act as a defense mechanism for the body to fight off infections. They also form the body's first line of defense, helping to combat bacteria and viruses that enter through the mouth and nose. Here is a brief overview of the anatomy of tonsils.

  • Location: Tonsils are situated at the junction of the oral cavity and the pharynx, on both sides of the throat. 
  • Types: There are three types of tonsils, that are palatine tonsils (these are the ones that most people are referring to when they mention their tonsils), adenoids (behind your nose), and lingual tonsils (at the base of your tongue).
  • Composition: Tonsils are composed of lymphoid tissue, which is made up of immune cells called lymphocytes. These immune cells help fight infections and protect the body from harmful pathogens.
  • Function: Tonsils are tiny masses of lymphoid tissue in the oropharynx. As part of your immune system, they help to fight off bacterial infection. They trap pathogens and produce antibodies against them.
  • Size: Tonsils range in size among individuals. Some tonsils may be larger and more noticeable, while others may be tiny and almost unnoticeable.
  • The risk of getting infected: The tonsils also get infected, a condition known as tonsillitis. Tonsillitis is characterised by the swelling or inflammation of the tonsils. Infections of the tonsils are either caused by bacteria or viruses. Symptoms include fever, sore throat, pain when swallowing, trouble swallowing, and runny nose.

Although the tonsils are a part of the lymphatic system, they are non-critical and do not compromise the ability of the body significantly to fend off infection. 

Different Types Of Tonsil Removal Surgery

Description

Traditional Tonsillectomy

Here the tonsils are completely removed. This procedure is done under general anesthesia.

Coblation Tonsillectomy

It uses radiofrequency energy to dissolve and eliminate the tonsil tissue with minimal damage to surrounding tissue, which could lead to reduced pain and quicker recovery as compared to traditional tonsillectomy.

Laser Tonsillectomy

Laser tonsillectomy uses a laser to remove the tonsils.  The laser beam is directed at the tonsil tissue, effectively vaporizing it and sealing blood vessels.

Partial Tonsillectomy

This surgical procedure includes only partially eradicating the tonsils, leaving some remnants behind. This method offers a faster recovery and less post-operative pain.

How Does a Tonsillectomy Work?

Before Surgery

  • Consultation: Patients will have a consultation with their healthcare provider, usually an ear, nose, and throat (ENT) surgeon. You can discuss the procedure, potential risks, benefits, and alternatives with your doctor.
  • Medical evaluation: He/she will discuss your medical history, carry out a physical examination, and go for additional tests if necessary. This helps assess your eligibility for the procedure.
  • Medication review: Inform the doctor about your current medications and supplements if any. You might need to adjust the dosage or temporarily stop certain medications such as blood thinners.
  • Smoking and alcohol use: You will be advised to stop smoking and use of alcohol for a brief period before and after the procedure as it can interfere with healing. 
  • Fasting: You will be asked to avoid food and drinks from midnight the day before the procedure. Follow any dietary instructions provided by the doctor.
  • Arrange help: Make sure that you arrange for someone to drop you home after the procedure. Also, arrange for help during your recovery.

During Surgery

  • Anesthesia: The patient is administered general anesthesia in order to make it painless.
  • Positioning: Once the patient is under anesthesia, they are positioned on their back with their mouth open, and a mouth gag or retractor may be used to keep the mouth open and provide access to the tonsils.
  • Tonsil removal: The surgeon uses surgical instruments to detach the tonsils from the surrounding tissues and removes them.
  • Hemostasis: After the tonsils are removed, the surgeon takes steps to control bleeding by cauterizing blood vessels or applying pressure to the surgical site.
  • Closure: The incisions are closed with the help of stitches or dissolvable sutures to promote healing.

After Surgery

  • Recovery room: Following the operation, the individual is transported to the postoperative recovery area. Here, the patient is kept under observation until he/she is stable.
  • Pain control: It is normal to experience some amount of pain and discomfort during the initial days after the surgery. This can be managed with prescribed medications.
  • Diet: Patients are generally advised to start with clear liquids and soft foods and gradually progress to solid foods. 
  • Rest and activity: Patients are supposed to take adequate rest in order to allow your body to heal. Also, make sure to avoid strenuous activities such as lifting heavy weights. 
  • Recovery Time: The majority of patients can expect to return to their normal activities, including school or work, within 1 to 2 weeks after surgery. However, full recovery may take several weeks
