Bladder neck suspension surgery is a surgical procedure used to treat stress incontinence in women. Stress incontinence is a common condition where laughing, sneezing, or coughing lets a few drops of urine escape from the bladder involuntarily. Bladder neck suspension involves lifting and securing the bladder and urethra to the pelvic bone. 

This prevents the bladder from leaking urine on coughing, laughing, or sneezing. This surgery is done in several ways. Open abdominal surgery, surgery through the vagina, laparoscopic surgery, and sling surgery are the various approaches to this surgery. 

What Is A Bladder Neck Suspension?

Bladder neck suspension surgery , or Burch Procedure, is a surgical way to treat stress incontinence in women. Stress incontinence happens when the muscles supporting the bladder and urethra are weakened. This causes urine to leak when the lady coughs, sneezes or laughs. Stress incontinence is commonly seen after-

  • Pregnancy
  • Multiple Childbirth
  • Menopause
  • Surgeries in the pelvic area
  • Problems with the bladder and urethra

The surgery involves lifting the bladder and urethra and securing them to nearby structures, mainly the pelvic bone. Bladder neck suspension surgery is of various types. Your surgeon will evaluate your case and choose the best surgery.

  • Open retropubic suspension- This is an abdominal procedure. The surgeon approaches the bladder and urethra through incisions made on your abdomen.

  • You can ask your surgeon to explain the procedure's details, outcome, and prognosis. You will be asked to sign an informed consent form agreeing to proceed with the surgery.
  • Discuss your medical history with your surgeon and inform him about-
    • All the medications that you are currently taking for your health conditions
    • Any dietary supplements and herbs that you are taking

    • Cystoscopy- To check for abnormalities in the urethra and bladder
    • Ultrasound scan of the bladder.
  • You will be asked to stop medications like blood thinners (to prevent excessive bleeding during and after surgery) a week before surgery.
  • On the day of the surgery, you will be asked to fast for at least 12 hours before the surgery.
  • You will be asked to arrange for a friend or relative to drive you home after the surgery.

What Happens During A Bladder Neck Suspension?

Bladder neck suspension surgery is done when the conventional treatment for stress incontinence has failed to yield the desired results. The surgery is performed by a skilled urologist and takes 60 to 90 minutes.

The surgery is done under general anesthesia. You will be advised to maintain a fasting period of a minimum of 12 hours. An IV line will be set up by attaching a catheter to a vein in your hand. This helps to keep administering fluids into your body. 

  • You will be given general anesthesia, and once the anesthesia sets in, the surgeon can perform the surgery through 3 approaches
    • Open retropubic suspension- This is an abdominal procedure. The surgeon approaches the bladder and urethra through incisions on your abdomen near the belly button.
    • Laparoscopic retropubic suspension- This procedure is done laparoscopically. Smaller incisions on your belly help the surgeon reach the bladder and urethra.
    • Needle bladder neck suspension- This procedure uses a vaginal approach to reach the bladder.
    • Sling Surgery- This bladder neck suspension surgery uses a sling or a hammock to cradle the bulging bladder. The hammock is usually created from a piece of your body tissue, mostly from the abdominal wall.
  • Once the lifting and securing of the bladder against the pelvic wall is complete, the incisions are closed with sutures.
  • You will then be transferred to a recovery room, where your vitals will be monitored. 
  • You will be kept in the hospital overnight. Once your pulse, blood pressure, respiratory rate, and temperature are normal and you have successfully passed urine and motions/gas a few times, you will be sent home.
  • The recovery depends on the type of surgery done. The open abdominal procedure takes 6 to 8 weeks. The laparoscopic and vaginal approach surgeries take comparatively lesser time to heal.

How Is The Recovery After a Bladder Neck Suspension Procedure?

Recovery from bladder neck suspension surgery depends on the type of procedure the surgeon has performed. The open abdominal approach takes 6 to 8 weeks to heal. Procedures done laparoscopically and through the vagina have a shorter healing period. Here is how you can care for yourself after the surgery and heal faster.

  •  Take all the prescribed medications on time
  • Drink adequate amounts of water to stay hydrated
  • Rest completely till the pain and discomfort go away

    • Strenuous exercises
    • Lifting heavy objects
    • Having sex
    • Using tampons and menstrual cups
    • Straining during bowel movements
