What is a Nephrostomy?

A nephrostomy is a procedure where a catheter (a thin, flexible tube) is placed directly into the kidney through the back. This allows urine to bypass a blockage in the ureter (the tube that carries urine from the kidney to the bladder) and drain directly into a collection bag outside the body. This procedure can be life-saving in situations where urine flow is obstructed, preventing kidney damage and other serious complications.

Why is a Nephrostomy Performed?

  1. Preparation:
    • Before the procedure, a thorough evaluation is conducted, including imaging tests such as ultrasound, CT scan, or MRI to determine the location and cause of the obstruction.
    • Blood tests may be performed to assess kidney function and overall health.
    • Patients are advised to fast for a few hours before the procedure and may be given antibiotics to prevent infection.
  2. Anesthesia:
    • Nephrostomy is typically performed under local anesthesia, which numbs the area where the catheter will be inserted. Sedation or general anesthesia may be used in some cases, depending on the patient's condition and the complexity of the procedure.
  3. Insertion:
    • The patient lies on their stomach or side. Using imaging guidance (fluoroscopy, ultrasound, or CT scan), the doctor identifies the exact location for catheter insertion.
    • A small incision is made in the skin, and a needle is inserted into the kidney. Once the needle is in place, a guide wire is passed through the needle, and the needle is removed.
    • The nephrostomy catheter is then threaded over the guide wire and positioned in the kidney. The guide wire is removed, and the catheter is secured in place with sutures or adhesive tape.
  4. Post-Procedure:
    • The catheter is connected to a drainage bag to collect urine. The patient is monitored for a few hours to ensure there are no immediate complications. Most patients can go home the same day, though some may need to stay in the hospital for observation.

The Nephrostomy Procedure

Recovery and Aftercare

Recovery from a nephrostomy involves several important steps:

  1. Activity Restrictions:
    • Patients are generally advised to avoid strenuous activities, heavy lifting, and bending at the waist for a few days to allow the insertion site to heal properly.
  2. Catheter Care:
    • Keeping the catheter site clean and dry is crucial to prevent infection. The dressing should be changed regularly, and the catheter should be flushed as instructed by the healthcare provider.
  3. Monitoring:
    • Patients should monitor the amount and appearance of urine in the drainage bag. Any changes, such as decreased urine output, cloudy or foul-smelling urine, or blood in the urine, should be reported to the healthcare provider immediately.
  4. Follow-Up Appointments:
    • Regular follow-up appointments are necessary to check the catheter’s position, function, and to decide on further treatment for the underlying condition causing the obstruction.

Risks and Complications

While nephrostomy is generally safe, it does carry some risks, including:

  • Infection at the insertion site or in the kidney
  • Bleeding
  • Damage to surrounding organs or tissues
  • Displacement or blockage of the catheter
  • Pain or discomfort at the insertion site
