  • Heaviness in the Scrotum: A feeling of heaviness or dragging in the scrotum.
  • Breast Tenderness or Enlargement: In rare cases, testicular cancer can cause hormonal changes that lead to breast tenderness or enlargement (gynecomastia).
  • Back Pain: Advanced testicular cancer that has spread to the lymph nodes or other areas can cause back pain.

Causes and Risk Factors

The exact cause of testicular cancer is not fully understood, but several risk factors can increase the likelihood of developing the disease:

  • Undescended Testicle (Cryptorchidism): Men with a history of an undescended testicle are at higher risk.
  • Family History: A family history of testicular cancer increases risk.
  • Age: Testicular cancer is most common in men aged 15 to 35.
  • Race and Ethnicity: White men are at a higher risk compared to men of other races.
  • HIV Infection: Men with HIV, especially those with AIDS, are at increased risk.

Diagnosis of Testicular Cancer

Diagnosing testicular cancer involves several steps:

  1. Self-Examination: Regular self-examination can help detect lumps or changes in the testicles early. Men are advised to perform monthly self-exams, especially if they have risk factors.

Treatment for testicular cancer depends on the type and stage of cancer and the patient’s overall health. Common treatment options include:

  1. Surgery:
    • Radical Inguinal Orchiectomy: The primary treatment involves the surgical removal of the affected testicle. This procedure is both diagnostic and therapeutic.
    • Retroperitoneal Lymph Node Dissection (RPLND): In some cases, lymph nodes in the abdomen are also removed to check for cancer spread.
  2. Radiation Therapy:
    • Used mainly for seminomas, radiation therapy involves high-energy beams to kill cancer cells. It is typically applied to the lymph nodes in the abdomen.
  3. Chemotherapy:
    • Chemotherapy uses drugs to kill cancer cells throughout the body. It is often used for non-seminomas and advanced testicular cancer that has spread to other parts of the body.
  4. Surveillance:
    • For early-stage testicular cancer, active surveillance (close monitoring with regular follow-up visits and tests) may be recommended after initial surgery.
  5. High-Dose Chemotherapy and Stem Cell Transplant:
    • In cases of recurrent or refractory testicular cancer, high-dose chemotherapy followed by a stem cell transplant may be an option.
