What are Anterior Cruciate Ligament (ACL) injuries

The anterior cruciate ligament (ACL) is one of four major ligaments in the knee that helps to stabilize the knee joint. An ACL injury occurs when this ligament is stretched or torn. This type of injury is common in sports that involve sudden changes in direction, such as soccer, basketball, and skiing.

Causes of ACL injuries

ACL (anterior cruciate ligament) injuries are typically caused by sudden changes in direction while moving or an improper landing from a jump. The injury can also occur as a result of overuse or improper training techniques. Factors that may increase the risk of an ACL injury include muscle weakness or imbalances, poor conditioning, and a history of previous knee injuries.

Risk factors for ACL (anterior cruciate ligament) injuries include:

  • Female gender. Women are 2-8 times more likely to suffer an ACL injury than men, possibly due to differences in muscle strength, muscle activation patterns, and hormonal influences.
  • Participation in certain sports. Athletes who participate in sports that involve cutting, pivoting, and jumping movements, such as soccer, basketball, and volleyball, are at an increased risk of ACL injury.
  • Previous knee injury. Individuals who have previously suffered an ACL injury are at a higher risk of re-injury.
  • Poor muscle strength, particularly in the quadriceps, hamstrings, and hip muscles.
  • Poor neuromuscular control can lead to improper mechanics during movement.
  • Poor balance and proprioception (the ability to sense the position of one's body in space)
  • Biomechanical factors such as increased knee valgus (knock-knee) and increased femoral anteversion (twisted thigh bone)
  • Obesity or excessive body weight.

Symptoms of an ACL injury

The symptoms of an ACL injury can include

  • Sudden and severe pain in the knee: This is the most common symptom of an ACL injury and usually occurs at the time of the injury.
  • A popping or snapping sound: Some people may hear a popping or snapping sound when the ACL is stretched or torn.
  • Knee Instability: An ACL injury can cause the knee to feel unstable or to give way when weight is placed on it. This can make it difficult to walk or stand.
  • Swelling: The knee may become swollen and tender within a few hours of the injury.
  • Loss of range of motion: The knee may become stiff and difficult to move after an ACL injury.
  • Bruising: Some people may develop bruising around the knee after an ACL injury.

It's important to note that not all people with an ACL injury will experience all symptoms and some people may not have any symptoms at all. If you suspect you have an ACL injury, it's important to see a doctor for a proper evaluation and diagnosis.

How ACL Injuries Diagnosed ?

ACL (anterior cruciate ligament) injuries are typically diagnosed through a combination of a physical examination, imaging tests, and other diagnostic tests.

  • During a physical examination, a healthcare professional will assess your knee for signs of swelling, tenderness, and instability.
  • Imaging tests that may be used to diagnose an ACL injury include X-rays, MRI (magnetic resonance imaging) scans and CT (computed tomography) scans. X-rays can be used to rule out any fractures, but they do not show ligaments.  MRI scans are considered the most accurate test for diagnosing ACL injuries, as they can clearly show the structure of the ligament and any tears or damage.

How ACL Injuries are Treated?

ACL (anterior cruciate ligament) injuries are treated through a combination of non-surgical and surgical methods, depending on the severity of the injury and the patient's individual needs and goals.

Nonsurgical treatment options include:

  • Physical therapy: to regain range of motion and strength in the knee, and improve balance and proprioception.
  • Bracing: to provide support to the knee and help prevent further injury.
  • Medications: to manage pain and inflammation.
  • Rest: to allow the knee to heal and avoid further injury.
  • Rehabilitation exercises: to regain knee function, muscle strength, and endurance.

Surgical treatment options include:

  • ACL reconstruction: to replace the torn ligament with a new one, usually taken from the patient's own body (autograft) or from a cadaver (allograft). This procedure is done by arthroscopy, which is a keyhole surgery.
  • Osteotomy: To correct any underlying mechanical issues that may have contributed to the ACL injury.

[Note: The choice of treatment will depend on the patient's age, activity level, and overall health, as well as the severity of the injury. In general, surgical treatment is recommended for patients who are physically active and want to return to sports or other high-impact activities, while non-surgical treatment may be appropriate for older patients or those with a less active lifestyle. Physical therapy is an integral part of both surgical and non-surgical treatment and is usually started soon after the injury to help reduce pain and swelling and improve function.]

Complications of an ACL Injury

Some possible complications of an ACL injury can include:

  • Stiffness and loss of range of motion in the knee.
  • Osteoarthritis is a degenerative joint disease that can occur as a result of an ACL injury.
  • Re-injury or tearing of the ACL graft, if surgery was performed to repair the injury.
  • Instability of the knee, which can lead to difficulty walking or standing.
  • Loss of muscle strength and control in the affected leg.
  • Chronic knee pain that aggravates movement.
  • Nerve injury, infection, or chronic pain.

How to Prevent ACL Injuries?

There are several ways to prevent ACL injuries, including:

  • Strengthening exercises for the quadriceps, hamstrings, and glutes.
  • Improving balance and proprioception (awareness of the position and movement of the body) through exercises such as single-leg squats and lunges.
  • Improving landing mechanics when jumping or cutting by practicing proper technique and using plyometric exercises.
  • Wear proper footwear and use knee braces or supports if recommended by a doctor or physical therapist.
  • Stretching the muscles in the legs, hips, and core to improve flexibility.
  • Gradual progression in sports and physical activity, and avoiding overuse.

It's important to note that preventing ACL injuries is a complex process, and different athletes may require different approaches depending on their sport, skill level, and individual risk factors.

What is the Prognosis for ACL Injuries?

The prognosis for ACL injuries can vary depending on the severity of the injury and the individual's specific circumstances. In general, the prognosis is generally better for younger and more active individuals, as well as for those who receive prompt and appropriate treatment. At Medfin we can guide you on the best and the most effective treatment methods for ACL injuries.
